BOTOX® is a therapeutic muscle-relaxing agent that works at motor nerve endings (nerves that lead to muscles). It is in a class of drugs called neurotoxins. When considering neurotoxin therapy, it is important to understand how the product works, the history of its use in patients, its protein content, and possible side effects. This page is designed to help you understand more about BOTOX®: what it is, how it works, and how it can help you. Refer to this page in the future should you have further questions about BOTOX® treatment.

 Q. What is BOTOX®? A. BOTOX® is a novel therapeutic agent derived from the bacterium, Clostridium Botulinum. Also known as Botulinum Toxin Type A, the brand BOTOX® is produced in controlled laboratory conditions and given in extremely small therapeutic doses. BOTOX® is indicated for the treatment of blepharospasm associated with dystonia in patients 12 years of age and above. BOTOX® is being investigated for several conditions associated with overactive muscle activity. Botulinum Toxin Type A is the most studied of the seven different serotypes of botulinum toxin (A, B, C1, D, E, F, G). Each serotype has different properties and actions. No two are exactly alike.

 Q. How does BOTOX® work? A. Normally your brain sends electrical messages to your muscles so that they can contract and move. The electrical message is transmitted to the muscle by a substance called acetylcholine. BOTOX® works to block the release of acetylcholine and, as a result, the muscle doesn't receive the message to contract. This means that the muscle spasms stop or are greatly reduced after using BOTOX®, providing predictable and reliable relief from symptoms.

BOTOX® is not a cure. For many patients, however, its effects have been dramatic - symptoms usually begin to dissipate within a few See Mechanism of Actionfor more information.

 Q. How is BOTOX® administered? A. BOTOX® is injected into the muscle. Your doctor will determine the muscle(s) in need of treatment.

 Q. Does the treatment hurt? A. A very fine needle is used for the one to three injections that are usually given per muscle. Some patients report minor and temporary discomfort from treatment.

 Q. When does BOTOX® start to work? A. Usually, you will see the effects of BOTOX® within three days. The maximum benefit is reached in one to two weeks.

 Q. How long does the effect last? A. Given its unique mechanism of action, BOTOX® offers sustained relief, dose after dose over the course of long-term treatment. The relief you will feel from a single treatment of BOTOX® will normally be sustained for approximately three months. You will notice a gradual fading of its effects. At this point you will return to your doctor for your next treatment. Usually, BOTOX® treatment is required only three or four times a year. Symptoms may vary throughout the course of the condition, and so the degree of relief and duration of effect varies from person to person. Consult your doctor, who has special knowledge about how to achieve the best possible results with BOTOX® for your individual case.

 Q. How long can I be treated with BOTOX®? A. Treatment with BOTOX® can typically be repeated indefinitely. BOTOX® has been used for over 10 years worldwide. Acceptable safety in long-term treatment has been well established. There are a number of factors that can impact the long-term usage of BOTOX®. These include: Setting appropriate expectations - Changes occurring with subsequent BOTOX® injections may be less dramatic than the first injection. Appropriate muscle selection - Identifying and injecting the affected muscle can be difficult, complicated by the changing pattern of muscle involvement and progression of the disorder. Adequate dosing - Changes in response may require dose adjustment. Minimizing exposure to neurotoxin complex proteins1 - Botulinum toxins contain proteins. In certain circumstances, when foreign proteins enter the body, the natural response is to form antibodies to the protein. When antibodies are formed, the effect may be that one is no longer able to respond to the therapy. High doses and frequent injections of botulinum toxin have been linked to the formation of antibodies. 1,2 Antibody formation with BOTOX® is rare. The likelihood of forming antibodies is reduced by having treatment no more frequently than about every three months. BOTOX® has approximately 5 ng of neurotoxin complex proteins per 100 unit vial, a relatively low amount of protein, which may help to further minimize the potential to form antibodies.

 Q. Is BOTOX® a new treatment? A. No. BOTOX® has been used for over 20 years in thousands of patients worldwide. The American Academy of Neurology, American Academy of Ophthalmology, and National Institutes of Health have recognized BOTOX® (Botulinum Toxin Type A) as a valuable treatment.

 Q. Is BOTOX® right for me? A. Ask your doctor if BOTOX® is the right treatment for you. BOTOX® should not be used during pregnancy, if you are nursing, or if you are taking certain medications. Only your physician can determine the best course of therapy. The effects of BOTOX® may be increased with the use of certain antibiotics or other drugs that interfere with neuromuscular transmission. Ensure that your doctor is aware of any current medications you are taking. If you have any questions regarding the use of BOTOX® treatment, please consult your doctor.

 Q. What side effects may be experienced when using BOTOX®? A. All medications have some side effects. With BOTOX®, side effects are usually transient and mild to moderate in nature. Some people notice temporary weakness of muscles or discomfort at the injection site. Based on a recent study of BOTOX® in the treatment of blepharospasm (n=98), drooping of the eyelid occurred in 20.8% of patients. Other side effects may include eye irritation, dry eye/tearing/light sensitivity (10%), and superficial punctate keratitis

Mesotherapy is a technique, invented in 1952 by Dr. Michel Pistor, where medication is injected into the mesoderm, the layer of fat and connective tissue under the skin.
Over the past 50+ years, an excess of 15,000 physicians world-wide have used Mesotherapy on a daily basis for a variety of purposes, including overall weight loss, spot weight reduction, hair loss, scar revision and wrinkle elimination.
Mesotherapy is also used to correct unevenness, lumps, or divots that develop after liposuction as well as cellulite reduction.

Cellulite is caused by bands of connective tissue that trap the fat and thereby causing the "dimpling" effect. The medications used in Mesotherapy consist of natural ingredients, plant extracts and safe metabolism boosters.
They weaken the connective tissue bands and dissolve the trapped fat, creating a smooth,
dimple-free appearance. The medications also melt the fat beneath the skin and shrink the fat cells. The fat dissolves and is carried through the bloodstream and excreted by kidneys and bowel.

In 1987, the French Academy of Medicine recognized Mesotherapy as an inherent part of traditional medicine. The International Society of Mesotherapy encompasses 14 countries throughout Europe and South America.
Using a very tiny needle (0.4 to 0.6 cm or the size of an eyelash), the medication is injected into the mesoderm painlessly. Patients have universally reported that the treatments cause less pain than a momentary, light pinch.

For cellulite, weight loss, and spot weight reduction, the medications are injected directly to the areas beingtreated. Treatments are administered weekly, and patients usually begin seeing results by the fourth treatment. After 10 treatments, most patients see a significant decrease in size, improved skin tone, and smoothing of cellulite.

Many patients develop unevenness, divots, or lumps after undergoing liposuction. These areas can be successfully treated with Mesotherapy. The scar tissue that forms under the skin after liposuction is directly targeted and dissolved. Patients see and feel smoothing of divots and lumps, and the unevenness resolves.

The combination of vitamins, herbal products and medication that is injected is tailored specifically for each patient to optimize results.

 What are Restylane™, Juvederm™, Radiesse™, Sculptra™ and Perlane™?

Restylane™, Juvederm™, Radiesse™, Sculptra™ and Perlane™ are fillers which can be injected beneath the skin to remove facial lines, fill depressed areas and actually lift the skin in some areas. These fillers can dramatically rejuvenate the facial appearance and are widely used today in anti-aging facial skin care.

What are Restylane™, Juvederm™, Radiesse™, Sculptra™ and Perlane™ made of?

These soft tissue fillers are FDA approved, liquid or gel medications that are injected beneath the skin to plump or fill an area of the facial skin which is depressed, such as a line, fold or area of atrophy. Soft tissue fillers are used to plump facial lines and wrinkles, to camouflage a fold of skin around the mouth, and to fill depressed areas such as the cheek that occur during facial aging.

The new fillers are hypoallergenic unlike older collagen type fillers used in the past. Most are a form of Hyaluronic acid (HA) which is a normal “ground substance” in the skin which helps maintain fullness and moisture in youthful skin. Some HA products are cross linked or have larger particle size for longer lasting results. Some, newer longer lasting fillers are made of other types of materials which I will discuss below. The ingredient of the common facial fillers is:

• Restylane™-Hyaluronic Acid

• Juvederm™
-Hyaluronic Acid-Cross-linked

• Radiesse™-Calcium Hydroxyl Apatite

• Sculptra™
- Poly-L-Lactic Acid

• Perlane™-Hyaluronic Acid-Large particle

How Are Restylane™, Juvederm™, Radiesse™, Sculptra™ and Perlane™ Used To Correct Facial Aging?

Soft tissue fillers are injected into the skin to add volume. Think of fillers as skin plumping agents. With aging the skin loses collagen and becomes lax. Especially in the mid face and cheek area the face can develop depressions and make the face look gaunt. Fillers can be injected into these depressions to make the cheek look more full and youthful.

Fillers are also injected into deep facial lines. The nose to mouth lines or nasolabial fold lines and marionette lines give the face an aged appearance. The fillers can be injected into the depth of these lines to plump the lines and camouflage the folds. Fillers can also be used to plump the aging lips:

Recently, larger filler injections have been done to plump the cheek and fill and lift the jowl in a procedure called the liquid face lift, with dramatic results.

Dermal Fillers - Radiesse, Restylane & Juvederm

Radiesse

Radiesse® dermal filler is made of calcium-based microspheres suspended in a water-based gel. It's injected into the skin with minimal if any discomfort and the results are immediate and long lasting!

Healthy young skin contains collagen that gives it volume, flexibility and strength. As part of the aging process, your own natural collagen breaks down, diminishing facial volume. This loss of elasticity and volume results in the wrinkles, facial lines and skin laxity that rob your skin of its youthful appearance.

Welcome Back Your Own Natural Collagen

The ideal way to achieve wrinkle reduction is by replenishing lost volume. With RADIESSE® wrinkle filler, you welcome back your own natural collagen to smooth out the signs of aging. That's because RADIESSE wrinkle filler's unique formulation stimulates collagenesis – the production of new collagen – which results in increased volume for natural looking wrinkle correction that may last a year or more in many patients. It's also why more and more cosmetic surgeons, estheticians, and other medical professionals are choosing RADIESSE wrinkle filler for long-lasting facial rejuvenation.

The RADIESSE wrinkle filler difference:

  • Stimulates the body's own natural collagen production
  • Instantly corrects wrinkles
  • Results that may last a year or more in many patients
  • Now FDA approved to enhance patient comfort1 with lidocaine mixing

In just one syringe, many patients can achieve long-lasting youthful results. And now you can ask your doctor about the RADIESSE treatment option that significantly improves comfort. Because treatment with RADIESSE wrinkle filler is simple, quick, comfortable and convenient, with immediately visible results that last, more and more people are discovering that it is the natural choice for maintaining a more youthful appearance.

Restylane

Why Restylane®?

Looking for non-surgical advancements in facial rejuvenation? Well, look no further. With the recent introduction of a revolutionary filler known as Restylane, patients in the United States now have access to a product that has been used in Europe for several years showing great success in achieving facial rejuvenation with injections. While there are numerous injectable fillers on the market today, many of them have their drawbacks restylaneincluding allergic reactions and relatively short life spans. Restylane is a safe and natural cosmetic filler that is immune to these drawbacks given its unique chemical composition. In December 2003, the U.S. Food and Drug Administration approved the use of Restylane for the treatment of facial wrinkles. Additional applications of Restylane include:
  • Lip augmentation and/or enhancement
  • Reducing the appearance of lines around the mouth
  • Modifying nasolabial folds (under the nose) and eye trough deformities
  • Facial contouring of the cheeks and mouth
It is important to note that while Restylane has been approved to enhance the appearance and fullness of the lips, the safety and efficacy of this application of Restylane has not been established by the FDA.

Collagen (CosmoDerm™ / CosmoPlast™)

COSMODERM® and COSMOPLAST® are two parts of Allergan's Collagen Replacement Therapy™ that rejuvenates the face and smoothes wrinkles. Approved by the U.S. Food and Drug Administration in 2003, they are the first and only human collagen dermal fillers approved for the treatment of fine lines and lip border.

Mimicking the natural effects of collagen, a natural protein produced in the body, COSMODERM® and COSMOPLAST® contain a highly-purified human collagen to fill in fine lines and wrinkles near the eyes and on the forehead, as well as for restoration of the lip border. COSMODERM® and COSMOPLAST® replenishing collagen lost due to aging, sun exposure, and the environment to restore the natural structure of the skin for a beautiful appearance. As the first FDA-approved dermal fillers to not require a pre-treatment skin test and formulated with lidocaine for patient comfort, they deliver immediate results with no down time.

  Perlane®

Perlane® is an injectable cosmetic dermal filler that has been used to soften wrinkles and facial folds of patients across the world. It is also a popular filler for lip augmentation, facial depressions, and facial scarring.

Perlane® is made from non-animal stabilized hyaluronic acid (NASHA) gel. This means that Perlane® contains no animal products. Since hyaluronic acid is a natural substance found in the skin and other tissue, there is no need for a skin test when using Perlane® filler.

The hyaluronic acid in Perlane® supplements the body's natural supply, which becomes depleted with age. Perlane® is biodegradable so its effects are not permanent. The results of a Perlane® filler treatment tend to last six months to a year.

Perlane® can be used in conjunction with BOTOX® Cosmetic, which relaxes the muscles that create wrinkles, but Perlane® simply reduces the visibility of wrinkles and folds.

What Areas of the Face Can Be Treated with Perlane®?

The hyaluronic acid in Perlane® can be used to improve the contour of facial skin and to reduce wrinkles, folds, scars, and other facial skin depressions.

Restylane® vs. Perlane® Filler

Perlane® is primarily the same product as  but Perlane® filler contains a larger concentration of hyaluronic acid. Because of this higher concentration of hyaluronic acid, Perlane® is usually used for larger folds, while Restylane® is better suited for smaller wrinkles or to maintain the results of a previous Perlane® injection.

Sculptra®/New-Fill™

Sculptra® (marketed as New-Fill™ in many countries) is a poly-L-lactic-acid-based injectable dermal filler. Sculptra® is very effective when used as a treatment for wrinkles and fine lines on the face, and it can also be used to improve facial volume and treat lipoatrophy, which is the facial wasting, or loss of facial fat, most often associated with HIV and AIDS.

Used throughout Europe under the name of New-Fill™ since 1999, Sculptra® is an injectable treatment made from polylactic (also referred to as poly-L-lactic) acid that was FDA approved in August 2004 for the treatment of facial lipoatrophy (facial wasting), especially in patients with HIV/AIDS.

The main ingredient of Sculptra®, polylactic acid, has been used for more than 20 years in surgical products, including dissolvable stitches. Polylactic acid is a synthetic substance made to replicate a natural derivative created by the muscles during exercise. Since Sculptra® is comprised of synthetic materials, there is no need for allergy testing prior to treatment.

After Injection, the polylactic acid will gradually promote the growth of new collagen, which, is not visually evident for approximately six to eight weeks, at which time the patient will return to the physicians office for a repeat procedure. As the patients collagen is stimulated, a fuller and more youthful appearance will take shape and continue for upto two years. Typical treatments take approximately 30 to 45 minutes per vial used, and results can last two or more years!
